What it is

Testosterone cypionate is a long-acting esterified form of bioidentical testosterone administered by intramuscular or subcutaneous injection.

How it works

Exogenous testosterone supplies the androgen that endogenously is produced by the testes (and, in smaller amounts, the adrenals). Therapy is typically considered for patients with documented clinical hypogonadism — low testosterone in the presence of associated symptoms.

What your provider will discuss with you

  • Recent labs: total testosterone (morning, fasting), free testosterone, estradiol, hematocrit, PSA, CMP
  • Prostate history and family history of prostate cancer
  • Cardiovascular history
  • Fertility plans (testosterone therapy suppresses fertility)
  • Sleep apnea, polycythemia history
  • Injection technique and frequency

Considerations

Published literature on testosterone therapy notes potential effects including increased hematocrit, mood changes, sleep apnea exacerbation, fertility suppression, and cardiovascular considerations. Ongoing monitoring (typically labs every 3–6 months in the first year) is standard.

Regulatory note. Testosterone cypionate is a controlled substance (Schedule III in the U.S.). Prescribing is subject to federal Controlled Substances Act rules and state-by-state telehealth controlled-substance prescribing rules, which vary materially by state.
