The FDA’s most serious warning (a Boxed Warning) applies to this class of medication: GLP-1 receptor agonists may be associated with thyroid C-cell tumors, including medullary thyroid carcinoma (MTC). Do not use if you or a family member have a history of MTC or Multiple Endocrine Neoplasia syndrome type 2 (MEN2).

What it is
Semaglutide is a synthetic peptide that acts as a GLP-1 receptor agonist. The compounded semaglutide available through Orbit Health is prepared by a U.S.-licensed compounding pharmacy partner and is distinct from FDA-approved branded products such as Ozempic® and Wegovy® manufactured by Novo Nordisk.
How it works
GLP-1 receptor activation influences appetite signaling, slows gastric emptying, and modulates insulin and glucagon release in response to glucose. Semaglutide is a single-pathway GLP-1 agonist; tirzepatide and retatrutide (investigational) are multi-pathway agonists by comparison.
Who your provider may consider it for
Your Orbit Health prescriber may consider compounded semaglutide for adults with elevated BMI or with metabolic risk factors after reviewing intake and any required labs.
What your provider will discuss with you
- Current BMI, weight history, prior weight-management approaches
- Personal or family history of medullary thyroid carcinoma or MEN2
- History of pancreatitis, gallbladder disease, or diabetic retinopathy
- Pregnancy, pregnancy plans, or breastfeeding
- Current medications, especially other diabetes medications or insulin
- Realistic expectations and follow-up cadence
Considerations
Published literature commonly notes gastrointestinal effects (nausea, decreased appetite, constipation) particularly during dose escalation. Your prescriber will review contraindications, monitoring, and discontinuation criteria with you.
Regulatory note. Compounded semaglutide is not FDA-approved. Availability is subject to FDA bulk-substance status and current FDA guidance regarding compounded GLP-1s. Compounded semaglutide is not Ozempic® or Wegovy® and has not been individually evaluated by the FDA for safety, efficacy, or bioequivalence to those branded products.
